Welcome to the Non-Permit Required Confined Space Entry Form
Before entering any confined space, it is essential to confirm that the space does not contain, and cannot develop, hazardous conditions that would require a permit. This form helps ensure that all evaluations, air monitoring, and entry precautions are completed and documented prior to entry. By carefully reviewing and completing each section, you help verify that the space is safe for entry under non-permit conditions. Thank you for taking the time to perform this verification and for maintaining a strong commitment to safety and compliance.
Purpose and Scope:
The purpose of this form is to provide written verification that a confined space has been evaluated and confirmed to have no actual or potential hazards capable of causing injury, illness, or death. This verification process ensures that the space meets all requirements for classification as a Non-Permit Required Confined Space (NPRCS). The form documents that atmospheric testing, hazard assessments, and rescue considerations have been completed, and that entry may proceed safely under non-permit conditions. Only trained and authorized personnel may enter a Non-Permit Confined Space.
An edit link will be emailed to the Entry Supervisor, allowing the supervisor to edit and modify this form as needed for adding personnel, updating monitoring results, making other authorized changes, and completing close-out verification at the conclusion of the work.
