Purpose and Scope:
The purpose of this permit is to provide a written authorization and checklist that ensures all hazards of a confined space have been identified, evaluated, and controlled before entry occurs. The permit documents that atmospheric testing, hazard controls, rescue planning, and worker training have been verified, and that only qualified and authorized personnel are allowed to enter. By using this permit system, employers and workers can maintain compliance with safety requirements and protect everyone involved in confined space work. An edit link will also be emailed to the Entry Supervisor, allowing the supervisor to edit and modify this permit as needed for adding personnel, updating monitoring results, making other authorized changes, and completing close out of the permit at the conclusion of the work.
