Purpose:
The purpose of this training is to help employers and workers understand the causes, symptoms, and prevention of heat illness. On multi-employer worksites, coordination and communication are critical. All employers share responsibility for ensuring their employees are protected from heat hazards. This training outlines strategies for prevention, roles and responsibilities, and proper emergency response.

Introduction

Heat illness can occur in both outdoor and indoor work environments where high temperatures, humidity, or radiant heat sources are present. OSHA does not yet have a specific heat illness standard, but employers are required under the General Duty Clause (Section 5(a)(1)) to provide a workplace free from recognized hazards. OSHA also enforces existing standards requiring potable water, sanitation, medical services, and PPE considerations that apply to heat.

Heat illnesses range from mild conditions, like heat rash and cramps, to severe emergencies such as heat stroke, which can be fatal. Every worker and employer must understand the risks and take preventive measures. On multi-employer worksites, host employers, contractors, and subcontractors must work together to ensure consistent protection for all employees.

Types of Heat Illness

Heat Rash

• Heat rash occurs when sweat ducts become blocked, trapping sweat under the skin.
• Symptoms include: clusters of red bumps or blisters, often in skin folds (neck, chest, groin, or elbow creases).
• While not usually dangerous, heat rash can be uncomfortable and may limit a worker’s ability to perform tasks safely.
• Treatment: Keep the skin dry, move to a cooler environment, and avoid wearing tight or non-breathable clothing.

Heat Cramps

• Heat cramps are painful muscle spasms that usually occur in the legs, arms, or abdomen during or after intense physical work in hot conditions.
• They are caused by loss of electrolytes (salts and minerals) through heavy sweating.
• Heat cramps are often an early sign of more serious heat stress.
• Treatment: Rest in a cool place, stretch gently, and drink water or electrolyte solutions. Avoid returning to strenuous work until cramps subside.

Heat Exhaustion

• Heat exhaustion is a serious condition caused by prolonged exposure to high temperatures combined with inadequate fluid replacement.
• Symptoms include: heavy sweating, weakness, dizziness, nausea, headache, rapid heartbeat, and confusion.
• If untreated, heat exhaustion can progress to heat stroke.
• Treatment: Move the worker to shade or a cooler environment, give small sips of water, and apply cool compresses. Monitor the worker closely.

Heat Stroke

• Heat stroke is the most severe form of heat illness and is a medical emergency.
• It occurs when the body’s temperature regulation fails and core body temperature rises above 104°F (40°C).
• Symptoms: confusion, loss of consciousness, seizures, very hot and dry skin, or excessive sweating that suddenly stops.
• Treatment: Immediately call emergency services. Move the worker to shade, remove excess clothing, apply cool water, ice packs, or wet cloths, and fan the worker. Do not leave the person unattended. Rapid cooling is critical to survival.

Prevention Strategies

Hydration

• Dehydration is the fastest path to heat illness. Workers should drink water before they feel thirsty.
• Best practice: One cup (8 ounces) every 15–20 minutes during heat exposure.
• Avoid caffeinated, sugary, or alcoholic beverages — these increase fluid loss.
• Employers should provide cool, clean, and accessible drinking water at all times.

Shade and Rest

• Rest periods in shaded or cooled areas give the body a chance to recover.
• Shade may be a tent, structure, air-conditioned vehicle, or naturally shaded area.
• Rest breaks should increase as temperature, humidity, and workload increase.

Acclimatization

• New or returning workers are most at risk of heat illness. The body needs time to build tolerance to working in hot environments.
• OSHA recommends gradually increasing exposure:
  • Start with 20% of usual workload on day 1.
  • Increase work duration by 20% each following day over 7–14 days.
• Supervisors must monitor new workers closely during acclimatization.

Work/Rest Schedules

• Modify schedules based on temperature, humidity, and workload.
• Heavy tasks should be done in cooler hours (early morning or evening).
• Rotate workers performing strenuous tasks.
• Allow more frequent breaks during extreme heat events.

Protective Clothing

• Wear lightweight, loose-fitting, breathable clothing to promote air circulation.
• Light-colored fabrics reflect sunlight better than dark ones.
• Hard hats with shade attachments, cooling vests, or neck wraps may be used in extreme heat.
• Avoid non-breathable materials such as rubber or plastic.

Multi-Employer Roles and Responsibilities

Host Employer

• Must establish a written Heat Illness Prevention Program (HIPP).
• Provide resources: water, shade, rest areas, and emergency response procedures.
• Communicate heat safety expectations clearly to all subcontractors.

Subcontractors

• Responsible for training their workers on heat illness awareness and prevention.
• Ensure compliance with the host employer’s HIPP.
• Provide supervisors to monitor employee conditions during hot work.

Shared Responsibilities

• Coordinate hydration schedules and rest breaks across all crews.
• Ensure shaded rest areas are available to every worker on site.
• Share emergency contacts and procedures so all employers respond consistently.
• Encourage all workers, regardless of employer, to report symptoms or unsafe practices immediately.

Recognize Symptoms Early

• Train workers and supervisors to watch for dizziness, confusion, slurred speech, or collapse.

Immediate Actions

• Move the worker to shade or a cooler environment.
• Loosen clothing and provide cool water in small sips (if conscious).
• Apply wet cloths, cool water, or ice packs to the armpits, neck, and groin.

Call Emergency Services for Heat Stroke

• Heat stroke can be fatal within minutes — call 911 immediately.
• While waiting for help, continue cooling measures.
• Never leave the worker unattended.

Recordkeeping and Continuous Improvement

• Training Records: Document completion of heat illness prevention training for all employees.
• Incident Reports: Record any heat-related illnesses or near misses to identify trends.
• Annual Review: Update the Heat Illness Prevention Program before the start of hot seasons.
• Worker Feedback: Encourage employees to share “good catches” and near misses related to heat so improvements can be made proactively.

Reporting and Resources

Reporting

• Report any signs or symptoms of heat illness immediately to your supervisor or designated safety lead.
• Notify site management if drinking water stations, shaded rest areas, or cooling measures are not available.
• Report near misses, unsafe conditions, or good catches through the OpsIntegrity

• Supervisors must document all reported incidents, including first aid cases and near misses, and ensure follow-up action is taken.
