    • This training program must be completed before the trainee is allowed to progress on to the 10-hour field training program.
    • This course must be completed with a score of 100% before the trainee can move forward.
    • The field training session requires instructor acceptance and approval before the trainee receives their completion certificate.

    • All computer based video training must be completed before entering the 10-hour field training program.
    • All tests must be completed with a score of 100%.

  • Competency Varification

    • The Trainee must log 10 hours of cab time in a loader in order to satisfy the field training requirements of the Loader Operator Training Certification.
      All 10 hours must be logged on the same ticket (this ticket).
    • Each time this ticket is submitted, an automated email will be sent to the Trainer/Mentor. The instructor can re-open the ticket by selecting “Edit Submission” link at the bottom of that email.
    • There is a 60-day limit on the completion of the Loader Operator Training and Certification course. If the Trainee does not complete the entire course during that time, they must start the program over from the beginning.
    • All fields of this form must be completed in order to satisfy the successful completion requirement of the 10-hour field training program.
